Here you will find, and may download, a selection of forms that you may need to complete from time to time.

Junior player registration

This is to be completed by all players for age groups up to U18. It must be signed by the player's parent or carer. A new form is required for each season. No player is allowed to play until this form has been registered with the Club Secretary. Managers please note that you should retain a copy of the completed registration form and pass the original to the Club Secretary.

Senior player registration

Adult players must complete this registration each season. No player is allowed to play until this form has been registered with the Secretary - Seniors Section. Managers please note that you should retain a copy of the registration form and pass the original to the Secretary - Seniors Section.

Manager application

All prospective team managers, assistants and coaches may apply for their roles by completing this application form. The completed form should be returned to the Club Welfare Officer.

Self declaration form

Completion of this form is compulsory for any appointed Club official who is in contact with children registered with the Club. The completed form should be returned to the Club Welfare Officer.

Parental consent

This form is to be completed by the parent or carer of any junior player if the player is about to take part in an activity such as a sports tour when the player's parent or carer will not be present. Managers please note that you should retain a copy of the completed consent form and pass the original to the Club Secretary.

Managers Income

Please use this form to record your monthly income. The completed form should be passed to the Treasurer along with your surplus funds each month.

Managers Expenditure

Please use this form to record your monthly expenditure. The completed form should be passed to the Treasurer along with your surplus funds each month.
